Are the big direct-to-consumer lab panels worth the money?

Short answer

Large direct-to-consumer panels are useful for a handful of standard tests and wasteful for the rest. Because reference intervals are set to include 95 percent of healthy people, running 60 independent tests on a healthy person makes at least one abnormal result close to certain. Many of these panels also include tests that guidelines advise against, such as reverse T3 and routine vitamin D screening.

Direct-to-consumer panels sell breadth. A typical package reports 50 to 100 analytes, presents them on a dashboard with colour coded ranges, and offers a subscription for repeat testing. Some of what they measure is genuinely useful and hard to get otherwise, particularly for people without insurance. The problem is the rest of the panel, and the way the results are framed.

The statistics of testing everything

Reference intervals are defined to contain the central 95 percent of results in a healthy reference population, with limits at the 2.5th and 97.5th percentiles. That design means 1 in 20 healthy people falls outside on any single test. Run 60 tests that behave independently and the probability that every one comes back inside its interval is roughly 5 percent. In practice the tests are correlated, so the exact figure differs, but the direction does not: a wide panel on a healthy person nearly always generates at least one red flag, and that flag is usually noise.

What these panels often include

Several standard packages contain tests that specialty guidance advises against. The American Thyroid Association states that in healthy, non-hospitalised people, measuring reverse T3 "does not help determine whether hypothyroidism exists or not, and is not clinically useful". The 2024 Endocrine Society vitamin D guideline recommends against routine 25-hydroxyvitamin D testing in generally healthy adults, and the US Preventive Services Task Force issued an I statement on screening asymptomatic adults in 2021. Food IgG panels appear on many wellness menus, and the American Academy of Allergy, Asthma and Immunology lists "Immunoglobulin G (IgG) testing for food allergy" among methods it considers not useful or effective.

CLIA certified does not mean clinically useful

Panels frequently advertise that they run in a CLIA certified laboratory. CLIA is a federal programme that sets quality standards for laboratories performing testing on human specimens; it certifies how the laboratory operates, not whether a given test answers a clinical question. Separately, the FDA states that direct-to-consumer tests for non-medical, general wellness or low risk purposes are not reviewed by the FDA before sale. A result can be analytically accurate and clinically meaningless at the same time.

If you use one of these services, choose the smallest panel that answers a question you actually have, and take the results to a clinician who will interpret them against your history rather than against a dashboard. Repeat any single abnormal result before acting on it. Do not let a subscription set your testing interval.

The clinical detail

The multiple comparisons problem is the core statistical issue. With n independent tests each having a 95 percent probability of falling inside its reference interval, the probability that all results are normal is 0.95 to the power n: about 46 percent at 15 tests, 21 percent at 30 tests and 5 percent at 60 tests. Correlated analytes on the same panel reduce the effective number of independent comparisons, so these figures are upper bounds on the false alarm rate rather than exact predictions.

The downstream cost is where the harm sits: repeat testing, imaging prompted by an incidental abnormality, and occasionally a procedure. Positive predictive value falls as pretest probability falls, so the same borderline result carries far less meaning in an asymptomatic self-referred person than in a patient with matching symptoms.
