Should the thyroid cancer warning on GLP-1s worry me?

Short answer
The boxed warning comes from rodent studies: semaglutide and tirzepatide caused thyroid C-cell tumours in rats, and whether this happens in humans is unknown. A Scandinavian cohort of about 145,000 GLP-1 users found no increased thyroid cancer risk. The warning still means an absolute contraindication if you or a close relative has had medullary thyroid carcinoma or MEN 2.
Every GLP-1 approved for weight management in the United States carries the same boxed warning, and it is worth reading what it actually says. The Zepbound label states that in rats, tirzepatide causes thyroid C-cell tumours, and that it is unknown whether the drug causes thyroid C-cell tumours, including medullary thyroid carcinoma, in humans. The Wegovy label uses the same construction for semaglutide in rodents. Unknown is the operative word.
Where the warning came from
Rodents have far more GLP-1 receptors on thyroid C cells than humans do, and lifelong exposure in rats produced C-cell hyperplasia and tumours. That finding triggered the boxed warning at approval. It has never been reproduced in human populations, and it may be a species-specific effect. Regulators did not wait for human evidence before warning, which is the correct approach for a drug intended for very large numbers of people.
What the human data now shows
A Scandinavian cohort study published in the BMJ in 2024 compared about 145,000 patients treated with GLP-1 analogues, mostly liraglutide or semaglutide, against about 290,000 treated with DPP-4 inhibitors, across Denmark, Norway and Sweden, with just under four years of average follow-up. It found no evidence of an increased risk of thyroid cancer. The authors were careful about the limits of that finding. As one of them put it, they could not rule out increased risk of certain subtypes in smaller patient groups, for example people with a high inherited risk of medullary thyroid cancer, who are advised against these drugs anyway.
What the warning means in practice
The contraindication is specific and absolute: personal or family history of medullary thyroid carcinoma, or multiple endocrine neoplasia syndrome type 2. Family history means a parent, sibling or child. It does not mean a grandmother with a benign nodule, an aunt with papillary thyroid cancer, or your own Hashimoto's disease. Papillary and follicular thyroid cancers, which make up the large majority of thyroid cancers, are not what this warning is about, and having had one is not a contraindication in the label. The boxed warning also asks patients to report a lump in the neck, hoarseness, difficulty swallowing or persistent shortness of breath.
Before you start, ask your relatives one specific question: has anyone in the family had medullary thyroid cancer, or a diagnosis of MEN 2. That single answer settles this for most people. If you have had a thyroid cancer of another type, take the pathology report to your prescriber rather than assuming you are excluded.
The clinical detail

The Scandinavian nationwide register cohort compared roughly 145,000 GLP-1 receptor agonist users with roughly 290,000 DPP-4 inhibitor users across Denmark, Norway and Sweden with a mean follow-up just under four years and found no increased incidence of thyroid cancer. Statistical power was insufficient to exclude an effect on medullary subtypes specifically.
