Is the GLP-1 pill as good as the injection?

There are now two GLP-1 tablets approved for weight management in the United States. Oral semaglutide 25 mg, marketed as Wegovy tablets, was approved in December 2025. Orforglipron, marketed as Foundayo, followed in April 2026 and is a non-peptide small molecule rather than a peptide, which is why it has no food or water restrictions. Both are once daily.

How much weight the tablets produce

In OASIS 4, 307 adults with overweight or obesity and without diabetes took oral semaglutide 25 mg or placebo for 64 weeks. Mean weight change was about 14 percent with the tablet against 2 percent with placebo, and 30 percent of the treated group lost at least 20 percent of body weight. An earlier trial of a higher tablet dose, OASIS 1, tested oral semaglutide 50 mg for 68 weeks and produced 15.1 percent against 2.4 percent. Orforglipron in the ATTAIN-1 programme produced 12.4 percent at 72 weeks among adherent participants, against 0.9 percent on placebo.

The administration problem with the semaglutide tablet

Oral semaglutide is a peptide, and peptides are destroyed by stomach acid unless absorption is engineered. The label is specific: take it on an empty stomach in the morning with no more than 4 ounces of plain water, swallow it whole, and wait at least 30 minutes before any food, any other drink, or any other oral medication. Missing that window reduces absorption. For someone who takes levothyroxine, which has its own empty-stomach requirement, the morning becomes genuinely complicated. Orforglipron has no such restriction, which is its main practical advantage.

If needle avoidance is the barrier that has kept you from treatment, a tablet is a reasonable first choice and the efficacy gap against injectable semaglutide is small. If you need the largest possible weight reduction, or you have a condition where the injectable has the outcome data, the tablet is a step down. Ask which of those two situations you are in before choosing on convenience alone.
