What does poor sleep do to appetite?

Short answer

Short sleep raises appetite through measurable hormone changes. In a randomized crossover study of twelve healthy young men, two nights of restricted sleep lowered leptin by 18 percent, raised ghrelin by 28 percent and raised hunger ratings by 24 percent, with appetite for calorie dense carbohydrate food rising 33 to 45 percent. Population data point the same way.

Poor sleep does not make you hungry in a vague way. It changes the two hormones that signal fullness and hunger, and it changes them in the direction that produces eating. Leptin comes from fat tissue and reports that stores are adequate. Ghrelin comes mainly from the stomach and drives the search for food. Short sleep lowers the first and raises the second, and the appetite that follows is aimed at particular foods.

What the laboratory study found

Spiegel and colleagues restricted twelve healthy young men for two nights and extended their sleep for two nights, holding calories and activity constant, and published the result in Annals of Internal Medicine in 2004. Leptin fell 18 percent. Ghrelin rose 28 percent. Hunger ratings rose 24 percent and overall appetite rose 23 percent. Appetite for calorie dense, high carbohydrate foods rose 33 to 45 percent, which is why sleep loss shows up as a specific craving rather than a general one.

What the population data show

Taheri and colleagues measured 1,024 people in the Wisconsin Sleep Cohort and published in PLoS Medicine in 2004. Compared with eight hours of sleep, five hours predicted leptin 15.5 percent lower and ghrelin 14.9 percent higher. Body mass index formed a U shape against sleep duration with its lowest point at 7.7 hours. That study is cross sectional, so it shows association rather than proof of cause, but it moves in the same direction as the controlled experiment.

What this does not prove

Leptin and ghrelin are not the whole story of eating after a bad night. The hormone shifts are modest in absolute terms, and appetite also rises because the waking day is longer, because reward circuits respond more strongly when tired, and because self monitoring gets worse. Sleep is a treatable input into appetite. It is not a substitute for the rest of a plan, and fixing it will not by itself produce the weight loss that a medication or a structured program produces.

The practical version is short. Protect seven to nine hours of sleep opportunity, hold the wake time steady within about an hour across the week, and get screened for obstructive sleep apnea if you snore, wake unrefreshed or have blood pressure that resists treatment. Track hunger against hours in bed for two weeks before you decide your appetite is a discipline problem.

The clinical detail

In the Spiegel protocol, sleep was restricted to 4 hours in bed for two nights and extended to 10 hours for two nights, with caloric intake and activity fixed, so the hormone changes cannot be attributed to eating more or moving less. Leptin fell 18 percent (P = 0.04) and ghrelin rose 28 percent (P less than 0.04). In Taheri's cohort, 74.4 percent of participants slept under eight hours, and mean body mass index rose from 31.3 to 32.4 as reported sleep fell from eight hours to five. Neither study establishes that lengthening sleep produces weight loss, which is a separate question with weaker trial evidence. In clinic, treat short sleep as a modifiable driver of appetite and adherence rather than as a weight loss intervention in its own right.
